June 17, 2014 fatality at Holcim Cement, Midlothian, TX–due to confined space violation. Cement tanker driver climbed into the middle tank of his tanker trailer. Cement got loaded in on top of him. By the time responders got to him, the damage was done and he was pronounded dead shortly after.
