I thought this video was a terrible animation about death, but when i watched all of it i have been inspired to follow every rule i see. I don't want to end up like the person in rule #8
On the rule 5 its actually not the workers that do the wrong. Its the gas tester, who "winks" the worker in, and assuming its safe. If I would be a judge, I would even charge the gas tester for murder or involuntary manslaughter. In this case, the best would actually be to issue a conditional PTW/Confined Space Cert, that says everything else checks up, its only the gas test left, and then make sure the contracted gas tester REFUSES to do a gas test unless a conditional PTW/CSC is present. This is something I also learned during my driving exam, that you should NEVER wink a car out from a unmonitored intersection, even if you have time to wait. The reason was that even if the intersection is safe from your point of view, it may not be safe for his point of view, and if you wink him out, the driver in the other car may belive you are 100% assured its safe, and drive, thus "overriding" his own safety check. Same with pedestrians. Even on pedestrian crossings with "give way"-sign, you should never wink out a pedestrian waiting, even if your car is fully stopped, because then the pedestrian could start walking when its not safe to do so. In "give way" crossings, you should just stop the car and not do anything more, and just wait for the pedestrian to take a own initative to cross. Same I think applies here, the gas tester should NOT wink them in, unless the gas tester checked that all other prerequistes are present, which could be solved with a conditional PTW/CSC, especially if its the worker itself who calls the gas tester. (Or it could be arranged that the workers itself NEVER calls a gas tester, instead it should be managed through supervisor).
